A 12-Month Support Program for Lasting Recovery
Life can feel overwhelming — especially when everyday stress, anxiety, depression, or trauma lead to increased reliance on alcohol or drugs. At Safe Harbor Charlotte, we believe hope and healing are possible. That’s why we’re proud to offer BREATHE, a comprehensive, SAMHSA-funded 12-month program designed to support individuals navigating Substance Use Disorders (SUDs) and co-occurring mental health challenges (COD).
BREATHE provides life-sustaining care that blends clinical services with community support — helping participants build stability, reduce disparities in access to care, and connect with resources that promote long-term recovery.
What Is the BREATHE Program?
The BREATHE Program is a year-long, comprehensive support system that helps individuals strengthen their recovery while building skills, connections, and resilience. This program goes beyond traditional treatment to meet people where they are offering support, structure, and community over time.
Key elements of BREATHE include:
✔ Enhanced Case Management — individualized support navigating treatment plans, appointments, and daily challenges.
✔ Behavioral Health Services — therapeutic support for mental health and substance use concerns.
✔ Recovery Housing Assistance — safe, stable living environments to support recovery efforts.
✔ HIV Intervention & Prevention Services — integrated care and education when clinically appropriate.
These components work together to help participants sustain recovery, build community, and access the comprehensive care they deserve.
Who the Program Serves
BREATHE is designed for individuals who are:
-
Navigating substance use disorders (SUDs)
-
Managing co-occurring mental health conditions
-
Looking for extended support beyond traditional treatment
-
Committed to building a life in long-term recovery
-
While the program is comprehensive, participation is currently managed through structured enrollment periods. Please check for updated openings or waitlist options.

Why BREATHE Works
A year-long model offers sustained engagement and prevents the “crisis-to-treatment-to-drop-off” cycle that many people experience. Participants receive consistent support, connection with professionals and peers, and guidance in life skills that extend well beyond the therapeutic setting.
Because BREATHE focuses on closing gaps in care, especially for underserved communities it helps promote equity in recovery outcomes and support for those who have historically faced barriers to quality healthcare.
